Post Acute Care is any skilled nursing and rehabilitation that occurs after a hospital stay. Typically, it serves to transition the patient to home assisting them in recuperation. It aids in improving care planning, and importantly, it increases positive communications between hospitals and the community-based providers.
We accept Medicare, Medicaid, private pay, and Insurance. The business office will have a list of insurances that the facility is currently in-network.
Yes, however, patients must be admitted after a three consecutive night hospital stay. Generally, during the first 20 days of care, Medicare will pay 100%. For days 21-100, a co-insurance rate is required. Medicare does not cover skilled nursing fees over 100 days.
Requests may be made for private rooms, they are not guaranteed. Based on our bed capacity situation we will make every attempt to meet your needs.

Yes, we staff on average 3 RN’s per every one of our 3 shifts. This provides a higher RN staffing ratio than the national average.
We have numerous full-time Nurse Practitioners and Medical Directors visiting the building on a daily basis to ensure high quality and satisfaction for our patients. Registered Nurses will be in constant contact with patients, and if requested, a patient can see a physician as often as needed.
Yes, provided that physical therapy is within the realm of the care plan.
We encourage family members to visit their loved ones anytime – 24hours, 7days, 365 days a year.
This is a non-smoking facility. However there are dedicated outdoor areas that residents are permitted to smoke at posted times.
Provided that the food is within the limitations set by our Registered Dietitian and as stated in the plan of care, the patient’s favorite food can be brought to them.
Private phone access is available to every patient as well as internet access. The patient can bring his or her own laptop; however, if needed we will provided one for our patients on a case by case basis and based on availability.
Depending on which room is selected your room may or may not be equipped with a television. Each room is equipped with a cable ready jack and coaxial cable is provided upon request for set up.
We have active patients that are committed to making their time enjoyable. Scheduled activities include: music, fitness activities, religious meetings, and art therapy.
